Express Check-In
For those of you eager to hit the water as
quickly as possible upon your arrival, you can print out
the check-in forms in advance and fill them out either before
you depart or while on the airplane. This form is valid for
any Dive Friends location on Bonaire. Simply click the link
below to access the two-page form, and then follow the directions
below:

Instructions--First Page:
1) At the top, please fill in your contact information
in the top box. If you are unsure of any information, leave
it blank, and inquire about it when you check in.
2) Sign and fill in your arrival date at
the bottom of the form on the first page.
Instructions--Second Page:
1) Read and check one of the two top boxes which differentiate
between certified and non-certified divers.
2) Read all remaining paragraphs and check each one as
you read and understand it.
3) Sign and fill in your arrival date at the bottom of
the second page. If you are under the age of 18, please
have your parent or guardian sign also.
PADI Medical Form
If you plan to take any courses or a Discover
Scuba Diving "Resort" Course, you will be required to fill
out a medical form to ascertain your fitness for diving.
If you answer "yes" to any questions on the form, you will
be asked to have a physician evaluate you for fitness. You
may download the form by clicking on the link below, and,
should you have reason to see a physician, can do so before
arriving on Bonaire. Follow the instructions on the downloaded
form.
